I was stationed in Germany. Battalion Armorer out on field training. 4-5 armorers slept and guarded battalion weapons and ammo. West Point Lt. comes in, problem with 1911 .45 pistol. If you cock it and push barrel in it shouldn’t fire if sear is functional. He claimed it wasn’t, pulled out gun, put hole in hand. Officers were given loaded guns. He forgot to unload first.
